1. A valid passport as well as a copy of its information page. Your passport must have at least six (6) months of remaining validity and at least one blank visa page in it.
2. One completed Visa Application Form
3. One passport photo (black & white or color is acceptable).
4. Formal application letter issued by the airline or shipping company.
5. The original and photocopy of the employee ID of the applicant.
6. Non-US citizens shall provide documents that entitle them to reside or work for an extended period of time in the US.

Note:

- Two years multiple entries  visas are issued to crewmembers of  Northwest, United , Continental, American, Fedex , UPS, Polar Air Cargo and Delta.
- To crewmembers of other airlines, single/double entry visas or 6 months /1 year multiple entry visas could be issued accordingly. 
- The duration of stay is normally 7-10 days. It should be specifically noted in the Application Form and the approval of the consul is necessary if a stay of more than 10 days is to be made.
